Urgent - Please help. We upgraded to Oracle database 12c since then one Oracle report is failing to

We upgraded to Oracle database 12c since then one Oracle report is failing to generate XML output with error Cannot view XML input using XSL style sheet. Please correct the error and then click the Refresh button, or try again later.

We tried different settings for XML Prolog value but no luck.

<?xml version="1.0" encoding="iso-8859-1"?>

<?xml version="1.0" encoding="&Encoding"?>

<?xml version="1.0" encoding="utf-8"?>

<?xml version="1.0" encoding="windows-1252"?>

Please advise.
