Unable to start MOS version of TFA 12.2.1.3.0 on single Linux host

Oracle support requested TFA collection for a different SR.  TFA cannot be installed during the 12.2 GRID or DB installation.   It must be obtained from MOS.  I downloaded and installed TFA 12.2.1.3.0.    I cannot start TFA.   I am not finding any related errors in the installation log file: /opt/tfa/ipht12/tfa_home/log/tfa_install_98868_2017_12_11-13_14_44.log.   I ran "/opt/tfa/bin/tfactl diagnosetfa ", but I am not sure what I am looking for in that output.  Doc ID 1513912.1 says to execute "tfactl diagnosetfa -local" and upload the resultant file to Support.   It does not state exactly how to get the file to support.  I am not finding a related problem type for creating a new SR.
