P4205 reducing quantity twice

Hi,

One of our client is facing issue like confirm ship happening twice.

One of the observation is user click OK and the screen does nothing. So user feels there is nothing happened, so clicked Ok button again. But it could have resulted into confirm shipment happened twice and item ledger has double entry. Also On hand quantity is now negative.

Does anyone had faced similar issue.
