How to include information in the header of a PDF report

Hello,

I would like to include in the header of a PDF report, which is generated in my application, the period that is provided by the user through two fields of type Date Picker.

Example: Report XXXX
             Period: xx / xx / xxxx to xx / xx / xxxx

How can I do this?
