batches posted at header level but not posted in F0911

Hi,

We have an issue since long wherein the batches seems to be posted at the header level but are unposted in F0911.

I am aware of the integrity reports to check and take care of such issues, but can somebody let me know the reason for such occurrence, so that we can fix it once for all and these issues doesn't come up?
