adop patch apply fails with Unable to connect to 'SYSTEM' even after it passes the validation. (afte

EBS - 12.2.5

We successfully applied 3 patches,  when we were applying 4th patches database got hung and end up restarted database.

Tried to apply 4th patch , said session already running.

Then i tried do update following "update ad_adop_sessions set status='C' where status='R;"  and tried running again.

adop phase=apply patches=27108180,27108180_ESA:u27108180.drv restart=yes abandon=no options=forceapply

adop phase=apply patches=27108180,27108180_ESA:u27108180.drv restart=no abandon=yes options=forceapply

adop phase=apply patches=27108180,27108180_ESA:u27108180.drv restart=yes abandon=no

No Success, patch apply failing on following issue:

Have attached the log details,

Wondering how come it gives system password error, even after it passed the validation of passwords successfully.
