What causes Fast Formula to stop being Verified?

We are using R12.1.3.

Today we hit a strange problem on PROD - when users tried to do a QuickPay, they got this error:

A compiled version of the Formula 123 cannot be found. Check that it exists and has been compiled before attempting to run it.

And when trying to submit an Absence via Employee Self Service, they got this error:

A compiled version of the Formula 539 cannot be found. Check that it exists and has been compiled before attempting to run it.

I could see that the Formulae were returned via this SQL:

     select ffft.formula_id
             , ffft.formula_name
