When entering a 1095c for a surviving spouse, the control number is blank for the 1095c

When entering a 1095c (manual) for a surviving spouse, and a COBRA employee, the control number is blank for the 1095c.  Not all surviving spouses are having the same issue however.  Last year we did have several with this same issue for manual entries, and ended up printing ONE 1095c with all the blank control numbers in Part III as covered individuals.

It seems like we fixed it last year, but I don't remember how.  Does anyone else have this same problem with blank control numbers being created?  And this is an issue in the print 1095c since several will print on 1 1095c incorrectly.
