1095C Forms Part 3 Dependents

I am building the 1095C forms and noticed I have some dependents printing in Part 3 in proper case (e.g. Jane Smith) instead of ALL CAPS as it is appears on the address book (both the tabs).  We set up the address book entries in ALL CAPS (e.g. JANE DOE).  I verified the address book, Who's Who entries, Dependent/Beneficiary entry and the name is listed in ALL CAPS.  What file does the 1095C build use for dependents?  Is there another file the dependent name could be stored on?  I only have a few dependents (on different employees) printing this way.
