CA Pensionable earnings and ER/EE amount contributed

We are running JDE 9.0 and 9.2.  history I need is from 9.0.  I need a report that shows me ytd earnings, pensionable earnings, and amounts contributed by both EE and ER.  Client would also like to see termination date and amount/rate of contribution.  What tables would I base that off of and how do I join to prevent duplication of information?  We use DAS for reporting.  This is time sensitive so any help is greatly appreciated.
