Create a new HTML page and capture response in Siebel

Hi All ,

We have to implement the below in our Project . It would be a great help if someone can give some ideas on how to proceed or has worked on a similar requirement .

1) Create a HTML webpage with Yes / No button and a free text field to store First Name  and DOB .

3) Host the webpage on a server ( like a normal website)

4)  Send an email to users containing the link of the above page

3) Capture the response provided in this webpage(by each of the user) in Siebel in some table .
