ACA - 1094C XML Schema Validation Failed - not well formed or missing required elements

Hello,

We are on version 9.2 and have submitted our 1094C files and were rejected.  Has anyone seen this error or could point me in the right direction to resolve the issue:

     AIRSH100

     XML Schema Validation Failed - not well formed or missing required elements  

     Form109495CTransmittalUpstream/Form1094CUpstreamDetail/Form1095CUpstreamDetail/EmployeeInfoGrp/MailingAddressGrp/USAddressGrp/AddressLine1Txt
