EBS R12.2.5 adop prepare phase failing with following error.

cat fndsvcrg.log

Cannot complete applications logon. You may have entered an invalid applications password, or there may have been a database connect error.

Internal Concurrent Manager status could not be determined.

Validated the Apps,Applsys and Weblogic passwords all are looking good and services are up and running without any issues, however PREPARE phase if failing.

Tried clean bounce of services and ran fs_clone with force option and ran autoconfig, but no luck.

Recent changes, Applied January 2018 PSU on RDBMS homes.

if anybody had faced similar situation, Please let me know highly appreciated.
