Cardex Not updating with the Order #

HI,

I need help/suggestion with one of the issues that I am facing right now.

I am using Item Reclassification application which is customized - we create IP transactions (Nothing but when we put roll into packages) IP Records gets created in the Cardex.

Randomly for some records DOCO and DCTO field in the cardex is not getting updated and since its not consistent unable to replicate the issue in lower environment and even i take logs, would not be beneficial since we dont know when its does not update the DOCO in Cardex. Since DOCO is not updated, few custom reports based on cardex giving incorrect results.
