Automatically using Media Object Templates

I have a need to automatically (using code) bring in a media object template when a new purchase order is created.  The users have a custom purchase order header form with a media object window in it.  When they create a new purchase order, they want a template to appear in the media object window.  The user creating the new purchase order will need to answer the questions contained in the template.  When the user saves the purchase order header, the text included in the media object window should be saved as a media object for the new purchase order
