Indicate Attachment Profile is set, but Icon still not showing correctly

Hi,

I have add the attachment facility to a form, and attached some notes to it. I have checked the profile 'Indicate Attachment' is set appropriately, but I still only get the paperclip and not the paperclip with the post-it note in it. Is there anything else that needs to set?
