EBS 12.1.3, Quick Pay and No Nacha file

Usually running via Quick Pay correctly creates the NACHA file, but sometimes nothing.  No output at all.  We void the payment and do over and it's fine.  Anyone else run into this?  And any suggestions on why and how to keep that from happening?
