Why aren't documents indexed in FIFO order from the DR$PENDING table

Using Oracle Text on Oracle 11.2.0.4/Windows 2008 R2.  We frequently see where the Oracle Text sync process does not index documents found in DR$PENDING in order based on PND_TIMESTAMP.  Thus, there are times when, after a sync, older documents are still left to be indexed.  Does anyone know why?
