Rebuild item - Instantiate

We have an item currently in use at the plant and has been identified as rebuild. Now I'm adding this on the Define Rebuildable Serial Number form but instantiate button is not enabled. Also the Parent Asset Number field is grayed-out. How do I properly set up a rebuild item for this case? Please see screenshot:
