PR Approval Process

Hello Team,

Not sure why we are getting the below error in approval cycle of PR, our PO approval hierarchy cycle is working fine. Please advice what setup is missing after reviewing the below screen shot. Approval status is stuck at 'In process' we are in Vision instance and not have done any customization.

Appreciate your assistance at the earliest.
