How to add a 11.2.0.4 database to cluster in OFS 3.4.2

Hi All,

 

We had oracle 11203 running on oracle fail safe (ofs) cluster 3.4.2. We removed from the cluster and upgraded the database to 11204. Now we are unable to add it back to the cluster. Can someone kindly send us a step-by-step guide to do this.
