Unable to submit request from request submission form due to the error "APP-FND-01242: cannot read v

Hi

After apply the patch 19287293 (CP CONSOLIDATED BUG FOR 12.1.3.2), we are unable to submit any requests from request submission form. When trying to submit, it throws this error:

"APP-FND-01242: cannot read value from field SET_REQUESTS.PARAMETERS"

Just tried compiling forms including pl/sql library, but no luck.

Any suggestion will be greatly appreciated.
