After a 12.2.1.2 reimage the ODAVP ran ok for a few weeks and now Storage is not accessible

Not sure I would use 12.2.1.2 (or the next version) as I reimaged using the VM (not bare metal) and my ODA is a paper weight. Ran ok for several days and then crapped out. None of the storage is accessible.

I have an SR opened now for several weeks.  Fortunately this was to be a DR server and is not yet production.
