How to map card related fees for CDF3 loading

Hi there, are there any experienced hands out there for MasterCard CDF3 mapping and loading?

We are live with PS expenses 9.2 (image 25) and have been utilizing PS delivered program to load MasterCard CDF3 files. All good EXCEPT that our users have reported that card related fees haven't been since go-live, including late fees, late fee credits and rush card delivery charges.

CDF3 file loading is new to us and the attached document details our configuration. We cannot figure out from the file (i have included some sample in my document) which is the merchant code for the banking fees. I tried to map 0165 to Delinquency fee and loaded a test file with such fees. Unfortunately nothing was loaded.
