CLOUD-0005: Can not connect. Check the status of the agent

In order to configure the db adapter in ICS, I downloaded a connectivity agent from OIC, then I installed it on DBCS host. The agent was started successfully and I was able to see the green light in agent monitor on OIC. I configured the connection by filling all required fields such as db host, port, username, port and agent group and when I tested, I got the following error displayed "CLOUD-0005: Can not connect. Check the status of the agent".you can find attached screenshots that explain further that error . please cloud you investigate the Reason for that error ?
