User-defined Exception Occurred In ap_vendor_sites_pkg.insert_row

Good Day!

We're trying to create a new vendor_site but see the error below.  There is personalization under the APXVDMVD form but the error still pops when custom code is off.  We accept the defaults for the fields as well as the accounting tab's gl_accounts. Does anyone have an idea about how to get past this?
