Profile: Service: Default Service Request Type

For profile Service: Default Service Request Type in Oracle.

When I change the "User Value" to something different than the Default value in the profile - in the Service Request HTML form, the User Value - Request Type does not pick up the value from the profile? Should this profile work on the HTML form?  Thanks!
