Custom Form Not able access after upgrade from 12.1.3 to 12.2.7

Hi,

We are upgrading from 12.1.3 to 12.2.7, As we already have custom form in 12.1.3, After upgrade to 12.2.7, Form is opening with error message as below. Once i click ok on error message, then form is opening. Can you please help on this issue?
