R12,1 clone issue FRM-40010: Cannot read form (points to prod path)

Hey everyone.  My luck with cloning lately seems to be going to ****.

R12,1.3 environment using separate db and app server configuration running RHEL 7.5.

I just run a couple of scripts 1 on the db and when that is finished 1 on the app tier and usually no real issues:

-  rman does the database

- adcfgclone dbconfig runs successfully

- adautocfg runs successfully

- rsync on the app tier runs to sync the app tier files

- I run a few other jobs

- adcfgclone runs successfully

- adautocfg runs successfully

In the end usually all good but this time when I go to launch Concurrent Requests it generates this error:
