Dear All, Can any one help me on generating Customer Statements in Release 12.2.6?

Dear All, Can you please please guide me in setting up the Customer Statements in Release 12.2.6

I followed the steps for configuring the Statement Cycle , Standard Messages, Aging Buckets. But not sure how we can get the Column headings and report headings. Following screen shot is the current report without modifying even the template. Thanks for your time.
