While we in ODA support have identified and dealt with this same problem in the past on a case by case basis we strongly recommend viewing the details in the following note

This is identified as Bug 23101338 - Disable diagsnap after 12.1.0.2.160419 GI PSU patch was installed (Doc ID 23101338.8)
