Error Testing Orchestration using Form service request

We are in E1 tools release 9.2.2.2 and recently installed Orchestrator studio version 6.1.0.0. I wanted to validate if it has been installed properly. For testing purpose, I created a sample Credit Hold Orchestration that calls Form service request to place the customer credit limit on Hold (same example that was done by Oracle), Using Orchestrator Client, I executed the Orchestration and received the following below error.
