9.2.1 Tools release OLE media objects are not working

We just went live with JDE tools release 9.2.1 and P4210 media objects (OLE) are not working as expected.  We are able to make the initial attachment, but when the SO Header is closed, and re-opened and we get the following error.

All users are getting this message on all attachments made after go-live.  All attachment's made before go -live are working correctly.  All users are on IE, and have current versions of Active-X.  Any ideas?
