Database user expired

Hi all,

Yesterday we have experienced a unique issue when suddenly a user account has expired in midday while the profile parameter PASSWORD_LIFE_TIME is set to UNLIMITED. Please take a look at below and advise on how to investigate what may have caused it and why ?

DEFAULT Profile:

As per my understanding, with the above configuration the user passwords should never expire however if login attempt with invalid passwords are made more than 10 times the account should lock. Please comment and let me know if more information is required.
