No NACHA File Output on a Quick Payment

Now and then on a Quick Payment we do not get an output file and no remittance advise is sent either.  It completes successfully but no output.  We've always been able to void the payment and then do another one with the same values and it works fine.  Any ideas on why this would happen?  It can be time consuming.
