Upgrading Oracle Database with a Physical Standby Database in Place

Hi, We are using four node RAC database with GI and GPFS v 4.2.2.2 with two node physical standby database . Database and GI version is 11gR2 (11.2.0.2). Now we are planning to upgrade GI +DATABASE to 12c. Need your help to clear some concepts and to understand the upgrade process both GI and Database. Please mention upgrade strategies with their fallback plan.
