Are all recommended patches MUST be applied in an on-prem environment?

Dear Experts,

As a best practice, should we apply ALL recommended patches in our on-premise environment? Or is it best that we just file for an SR to address our issues as they occur in each environment?

My only concern is that we might be paralyzed to test something that's not even applicable to our environments.
