FRM-92101 forms error happening randomly on AIX, creating java core dump and trace files

R12.2.5 AD/TXK Delta 7, AIX 6.1 x64

Once per week we shut down all application and db tier services and bring them back up.  In other words, we perform weekly system bounces so we have 52 uptime sessions per year.   We have about a 10% chance of the following scenario happening immediately after bringing up services and will continue for the remainder of the uptime session:

An end user clicks a java forms link within the EBS application, and is presented with the following error:

Then the user re-clicks the same link and forms may launch normally with 2-5 more attempts.
