WO inventory issues deducts on hand qty twice in E9.1

Hi

One of our client is experiencing this issue in E9.1 (tools release E9.1.5) . System randomly deducts on hand qty twice (F41021) whenever user does WO inventory issues thru P31113. We are using lot controlled items and it is happening randomly and could see records in F4111, but on hand gets depleted twice in F41021.  We are not able to replicate this issue in test environment and not identified the root cause. Any user reported this issue in E9.1 or known bug in E9.1 ?
