FRM 40654 & 40400 submitting concurrent requests

Hi - We patched our system this weekend and are now getting FRM-40654 and FRM-40400 when we try to submit various concurrent requests.  Has anyone else seen this behavior?  We're on 12.2.6 EBS, DB 11.2, and Solaris OS.  I don't see anything about this in the ReadMe's for the patches installed, so I'm not sure if it's a bug or possibly a default setting somewhere.  Does anyone have any suggestions?
