Verifying Shared Storage Discovery ...FAILED (PRVF-4100)

Hi

Runclusterverify.sh is failing with the below message while  Setting up  2 node 12cR2 RAC on Linux 7.x,

Verifying Shared Storage Discovery ...FAILED (PRVF-4100)

also, we labeled the shared disks using ASM filter driver (ASMFD) as below..

DATA01

DATA02

FRA

But after some time DATA01 disk is disappeared.

I would appreciate any help.
