The webpage cannot be found When Uploading File

Hi Experts,

Our customer is on EXA on Demand of R12.1.3. When they upload a file to an EBS application -> Select the file from local drive(C: or ) and click Submit button they get WEBPAGE CANNOT BE FOUND http 404 error. They don't have issues on other environment with the same build. I can't seem to find the problem(configuration, security??). Any input is appreciated.
