Unique Constraint violation (APPLSYS.FND_SESSIONS_U1)

We get intermittent and seemingly random occurrences of the above error when updating element entries.  We have never been able to determine the circumstances or conditions which cause it, so I wondered if anybody else has had the same issue and been able to solve it?
