Oracle Database 11g Release 11.2.0.1.0 - 64bit Production, platform Linux x86 64-bit

When executing the following insert statement, session was terminated suddenly. And we see the following line in alert log:

Exception [type: SIGSEGV, Address not mapped to object] [ADDR:0x8] [PC:0x5F9AAC3, kkqvSearchLogOpn()+29] [flags: 0x0, count: 1]

    Errors in file /usr/app/oracle/diag/rdbms/lims/LIMS/trace/LIMS_ora_39543.trc 
    (incident=29004):
