Trying to create Activity Workbench and saving it,getting an error as "Exception occurred"

Hi Folks,

I am trying to create the Activities from "Activity Workbench"

When trying to save the data in Activity workbench,we are getting a note error as "Exception occurred" and "ORA-00928:missing select keyword"

FRM-40508:Oracle error:unable to insert record

Can anyone came across this issue.
