Quality plan result missing

Hi All - I created a quality plan that requires a result to be entered before a work order can be completed when the WO has an activity number present.  This is as close as I can get to making a quality plan mandatory for completion of generated preventive maintenance WO's.  The plan has been active for about a year and is working well.  Now it appears that several WO's with activity numbers are in the "Complete" status but there is no quality plan result on the WO.  The WO's were created after the plan was enabled.

I'm trying to determine if this is a bug or User error - I haven't been able to recreate the issue.   Has anyone encountered this before?
