EBS upgrade to 12.2.6, can't login EBS URL

Hi,

upgrade path:

11.5.10.2>12.0>12.6

cannot login to EBS URL (sysadmin) after upgrade to 12.2.6:

if enter correct password, error show as below:

Unexpected URL parameters have been detected and will be ignored.

if enter wrong password, error show as below:

Login failed. Please verify your login information or contact the system administrator.

For enter correct password, accesslog message show as below:
