Steps to upgrade a database from 11.2.0.3 to 11.2.0.4

Hi,

Does anyone have a tried and tested steps that would lead me to a successful upgrade of the database from 11.2.0.3 to 11.2.0.4?

I already installed the 11.2.0.4 binaries under new home. I'm trying to use DBUA utility but this bounces from either the database is not mounted or not open or not open using UPGRADE.

Any help would be appreciated.
