Error while creating order from Parts Requirement form

We are facing following error while creating parts order, when checked one of the Item "ASY-11744" is not mapped to the Item Validation Org defined in the Operating Unit of Destination Organization. However, irrespective of the missing Item Validation Org setup to the Item "ASY-11744", we are able to create parts order in Test instance but not in Production environment.

Error: ORA-01403: no data found in Package OE_PROCESS_REQUISITION_PVT Procedure Update_Internal_Requisition.

Can anyone suggest the possible causes of the issue?
