FSG Report how to display Column Note with Number

Hi Team,

I have a requirement to configure FSG Report, with column NOTE as shown in below screen shot, i was able to bring the columns Revenue Category, 30th june 2017 and 1st july 2016 columns data. could any one guide me how to bring this column NOTE.
